Why this matters
A nursing home cited for medication handling needs pharmacy help. One getting a big share of its nursing hours from agency staff is already buying staffing. A new owner reviews its vendors. All of it is public, but it's spread across thousands of state and federal records, filed weeks apart, and nobody sends an alert.
We re-check those records on a recurring cycle, keep the history, match each citation to the kind of vendor that fixes it, attach the facility's administrator and phone, and resolve per-property LLCs to the operators behind them.

How we handle the data
Dates stay visible
Every row carries the event date from the state record. A fresh import is not a newer event, and we never relabel an old record as new.
Records read correctly
Most “new licenses” are existing buildings under new owners, and ownership changes are filed alongside fines. We sort each record into what actually happened.
Limits stated plainly
Phones are the facility’s main line as listed with the state. Nursing home citation data runs behind real time. New-administrator history starts from when our tracking began.
